HOMES Program
Housing Assistance, Housing Case Management, and Support
For single adults who are homeless or at risk of becoming homeless, the HOMES (Help, Opportunity, Move On, Evidence-Based Service, Support) program can provide assistance finding, securing, and then maintaining housing.
Services Offered
The HOMES program can help participants:
- Identify, determine eligibility for, and apply for housing and rent subsidy programs
- Search for appropriate available housing options
- Prepare for housing interviews
HOMES also provides post-placement support for six months after move-in, ensuring a smooth and successful transition for the client.
Eligibility
To enroll in HOMES, single adults must be:
- Currently homeless or at imminent risk of becoming homeless
- Enrolled in a Health Home, or eligible for enrollment with:
- Active Medicaid coverage
- At least one of the following:
- two or more chronic conditions (substance use disorder, diabetes, heart disease, etc.)
- HIV/AIDS
- a diagnosis of serious mental illness
