    HOMES Program

    Housing Assistance, Housing Case Management, and Support

    For single adults who are homeless or at risk of becoming homeless, the HOMES (Help, Opportunity, Move On, Evidence-Based Service, Support) program can provide assistance finding, securing, and then maintaining housing.

    Services Offered

    The HOMES program can help participants:

    • Identify, determine eligibility for, and apply for housing and rent subsidy programs
    • Search for appropriate available housing options
    • Prepare for housing interviews

    HOMES also provides post-placement support for six months after move-in, ensuring a smooth and successful transition for the client. 

    Eligibility

    To enroll in HOMES, single adults must be:

    • Currently homeless or at imminent risk of becoming homeless
    • Enrolled in a Health Home, or eligible for enrollment with: 
      • Active Medicaid coverage
      • At least one of the following:
        • two or more chronic conditions (substance use disorder, diabetes, heart disease, etc.)
        • HIV/AIDS
        • a diagnosis of serious mental illness
