Safe Options Support (SOS) Teams
Safe Options Support (SOS) is an innovative new (Spring 2022) program model funded through the New York State Office of Mental Health. The SOS teams provide comprehensive community care to People Experiencing Homelessness (PEH), with a focus on assistance in securing housing. SOS teams, which are staff by trained mental health clinicians and Peer Specialists, provide intensive care coordination and support to PEH, many of whom are living with a behavioral health condition. By partnering with emergency shelters, outreach providers, and supportive housing programs, the SOS teams support individuals as they transition from homelessness to stable housing.

Services Offered
SOS provides 24/7 call coverage, so participants have access to care at all times. In addition to a comprehensive assessment of client needs, services include:

Linkage to Medication-Assisted
Treatment for Substance Use
Entitlement Support
Housing Support
Behavioral Health Counseling
Time limited model to help support skill building
